If you offer prescription drug coveragethrough SummaCare, your employees can use our drug search, find copay information or an in-network pharmacy and learn more about SummaCare’s mail order pharmacy service.
Dental Benefits
Guardian Dental
Employers with dental benefits through Guardian Dental should call Guardian’s Customer Service Team at 800-627-4200 with questions and comments.
The Dental Care Plus Group (DCPG)
Employers with an Ohio DentaTrust, DentaSpan or DentaSelect Plus plan through DCPG can find a dentist, login to the portal and learn more about DCPG plans and benefits here. Members with Ohio DentaTrust or DentaSpan can call 855-343-4263 and members with DentaSelect Plus can call 800-367-9466.
Self-Funded
Employers with self-funded dental benefits through SummaCare should call their Account Manager with questions and comments.
Vision Benefits
EyeMed
Visit EyeMed Vision Careto view benefit information, order ID cards and locate a provider.
VSP
SummaCare’s small group and individual health insurance plans include pediatric vision benefits for children up to age 19 through our partnership with VSP, a national leader in vision and eye care benefits. Find a VSP provider todayor call VSP Customer Service at 855-868-4561.
Summary of Benefits (SBC)
SBCs are mandated under the Patient Protection and Affordable Care Act (PPACA) and the SBCs provided by SummaCare will reflect medical and pharmacy benefits only. They will not reflect or include details about Health Reimbursem*nt Accounts (HRAs), Health Savings Accounts (HSAs) or other ancillary benefits. SBCs for tailored benefit plans are available upon request from your Account Manager. Search by plan for the document you need.
