Experts are sounding the alarm about the dangers of cosmetic face fillers, highlighting the potential for serious complications like skin loss and even blindness. Researchers have been using ultrasound technology to examine cases where filler injections went wrong. The results are concerning, and clinics are now being advised to use these scans to avoid accidentally injecting fillers into or near blood vessels. But here's where it gets controversial: Dr. Rosa Sigrist, the lead researcher, explains that these 'vascular occlusion' events can be catastrophic, leading to tissue death and facial disfigurement if not treated promptly.
So, what exactly are dermal fillers? They're injectable substances designed to smooth wrinkles and rejuvenate the skin. They're also sometimes used to reshape features like the nose or lips. The area around the nose is particularly risky, as the blood vessels there connect to critical parts of the head. Damage to these vessels can lead to severe issues, including skin damage, blindness, and even stroke.
Dr. Sigrist's team, which includes researchers from the University of São Paulo in Brazil, studied filler-related vascular complications in 100 patients across several radiology, dermatology, and plastic surgery centers between May 2022 and April 2025. Their findings, presented at a medical conference, revealed some alarming details. In almost half the cases, ultrasound scans showed absent blood flow in small facial blood vessels. And in a third of the cases, major blood vessels were affected.
To prevent these complications, Dr. Sigrist recommends that clinics use ultrasound to plan and guide injections. Ultrasound can also help treat complications if they arise. This is a significant shift in practice. Rather than injecting a drug called hyaluronidase to dissolve the filler blindly, clinicians can use ultrasound to target the exact location of the blockage.
While the use of ultrasound is increasing, it's not yet standard practice. The British Association of Aesthetic Plastic Surgeons (BAAPS) notes that ultrasound is non-invasive and has no known harmful effects. Nora Nugent, president of BAAPS, emphasizes the value of mapping blood vessels before treatment.
This raises a critical question: why aren't more clinics using this technology? The BAAPS has been advocating for increased regulation of aesthetic procedures, suggesting that only medically trained professionals should administer injectable treatments. The UK government is also planning to introduce restrictions on cosmetic procedures, requiring clinics to meet strict standards and limiting high-risk procedures to qualified healthcare professionals.
